How I approach back pain

My approach is deeper than simply treating someone's back and trying to get a short term reduction in symptoms.

I work with the whole person, not just the painful area. I want to understand what is going on for them physically, but also the wider factors that can influence their pain experience, things like stress, sleep, lifestyle, habits, activity levels, previous experiences with pain and protective behaviours.

I'm trying to understand the bigger picture and what might be keeping someone stuck, rather than just treating the symptoms in isolation.

I don't just want to crack your back

Hands on treatment can have a place, and I'll use it where I think it will be helpful. But I don't want someone to become dependent on treatment every time their back hurts.

I want to understand the person in front of me

Work out what is influencing their pain and help them make meaningful changes they can actually carry forward.

Common questions

How long is an appointment and how many will I need?
The first appointment is long enough to take a full history, examine properly and explain what we found before you leave, rather than a rushed slot. Most people notice meaningful change within two to six weeks for a straightforward episode. You will get an honest estimate at the assessment rather than an open ended plan, and if you need fewer sessions than expected we will say so.
How far is the clinic if I am coming from Medway?
The clinic is in Rainham, roughly ten minutes from Gillingham and fifteen to twenty from Chatham, Rochester and Strood. There is dedicated parking directly outside, which matters more than it sounds when your back is bad and you are circling a town centre car park.
Massage helps for a few days then it comes back. Why?
Massage reduces muscle tone and feels good, but it does not change how much load your back can tolerate. If nothing else changes, the tissue is back where it started within a week. Hands on work still has a place here, but as a way to make the loading part possible, not as the treatment on its own.
How long does back pain take to settle?
Most straightforward episodes improve meaningfully within two to six weeks with the right plan. Longer standing pain takes longer, but you should understand what is happening from the first session.
Do I need a scan first?
Usually not. Scans often show changes that are entirely normal for your age and do not correlate well with pain. Josh is trained to identify the small number of cases where imaging genuinely changes management.
Should I rest or keep moving?
Almost always keep moving, within sensible limits. Prolonged rest tends to make back pain worse. Part of the assessment is working out what you can safely keep doing.
Do I need a referral?
No. You can book directly without seeing a GP first, which is what most people do. If you are claiming on private medical insurance, check your policy first because some insurers still require a GP referral before they will authorise treatment.
Can you help if I have had back pain for years?
Yes, and it is a large part of what we do. Persistent pain needs a different approach to a fresh injury, which is exactly what the assessment is for.
