How I approach shoulder pain

I see a range of shoulder problems, including rotator cuff related shoulder pain, rotator cuff tears, shoulder instability and dislocations, and frozen shoulder.

There isn't a one size fits all treatment for shoulder pain. Everything is based around what we find during the assessment and what you need.

I'll look at how the problem started, what you're struggling with, your movement, strength and loading capacity, as well as the wider factors that may be influencing your symptoms.

Common presentations

There isn't a one size fits all treatment. Everything is based on what we find in the assessment.

Rotator cuff related pain

The most common presentation. Usually a loading and capacity issue that responds well to graded strengthening.

Rotator cuff tears

A finding on a scan does not automatically explain symptoms. Many are found in people with no pain at all.

Needs accurate assessment and a properly structured return to activity and sport.

Frozen shoulder

A distinct condition with recognisable stages. Getting the stage right matters enormously.

Rebuild the cuff, restore the movement

Progressive strengthening of the rotator cuff and surrounding musculature. This is the part that produces lasting change.

Common questions

How long is an appointment and how many will I need?
The first appointment is long enough to take a full history, examine properly and explain what we found before you leave, rather than a rushed slot. Most people notice meaningful change within six to twelve weeks of progressive loading. You will get an honest estimate at the assessment rather than an open ended plan, and if you need fewer sessions than expected we will say so.
Do you offer physio for shoulder pain?
Yes. Physio for shoulder pain covers rotator cuff treatment, physio for torn rotator cuff, shoulder impingement treatment and frozen shoulder physio. Frozen shoulder is staged first, because the wrong treatment at the wrong stage makes it worse.
Do I need a scan before you can treat my shoulder?
Usually not. Shoulder scans very often show rotator cuff changes in people with no pain at all, so a scan on its own rarely tells you what is causing your symptoms. Clinical testing gives a clearer picture, and we will tell you if imaging would genuinely change the plan.
Is my shoulder pain a rotator cuff tear?
Many rotator cuff changes are found in people with no pain at all, so a tear on a scan does not automatically explain symptoms. Most cuff related pain improves substantially with structured strengthening.
How long does frozen shoulder take?
It follows a recognised course that can run twelve to eighteen months untreated. The right treatment at the right stage improves both comfort and range considerably along the way.
Why does it hurt more at night?
Shoulder pain frequently worsens at night due to sleeping position and reduced distraction. It is common and it is addressable.
Will I need an injection?
Sometimes an injection helps create a window to rehabilitate. It works best alongside a loading programme rather than instead of one.
Do I need a referral?
No. You can book directly without seeing a GP first, which is what most people do. If you are claiming on private medical insurance, check your policy first because some insurers still require a GP referral before they will authorise treatment.
