The starting point

How I approach neck pain

I commonly see desk workers, people with headaches, stress and tension related symptoms, and people who have been dealing with persistent neck pain for a long time.

As with everything I do, I don't look at the neck in isolation. I want to understand the story behind the problem and consider movement, strength, lifestyle, sleep, stress, habits and the person's beliefs and behaviours around their pain.

I'll also assess for symptoms that could indicate something requiring further investigation, such as symptoms travelling into the arms, pins and needles, numbness, weakness or other neurological signs.

Common drivers

There isn't a generic treatment that works for everyone. The approach is based on what I find and what you need.

Postural and work related

Sustained positions at a desk or in the car load the neck and upper back in ways they are not built to tolerate all day.

Joint and muscular

Facet joint irritation and deep muscular tension produce genuine local pain and stiffness, often worse first thing.

Cervicogenic headaches

A large proportion of persistent headaches originate in the upper neck and respond very well to treatment aimed there.

Whiplash and trauma

After a road traffic accident or fall the neck usually needs graded reintroduction of movement rather than protection.

Common questions

How long is an appointment and how many will I need?
The first appointment is long enough to take a full history, examine properly and explain what we found before you leave, rather than a rushed slot. Most people notice meaningful change within two to four weeks for simple mechanical neck pain. You will get an honest estimate at the assessment rather than an open ended plan, and if you need fewer sessions than expected we will say so.
My neck pain came on with no injury. Is that normal?
Very. Most neck pain has no single moment behind it. Sustained postures, workload, stress and sleep all change how tolerant the neck is, and the pain often appears once tolerance drops below what an ordinary day demands. That is diagnosable and treatable, it just is not an injury.
Can neck pain cause headaches?
Yes, very commonly. Cervicogenic headaches originate in the upper cervical spine and often present as pain at the base of the skull spreading forward. They typically respond well to treatment directed at the neck.
How long will it take?
Simple mechanical neck pain often improves within two to four weeks. Longer standing or post trauma cases take longer and benefit from a structured plan.
Should I use a special pillow?
Pillows help some people and make no difference to others. It is worth addressing but rarely the whole answer.
Do you treat whiplash?
Yes. Graded, guided reintroduction of movement is generally far more effective than rest or immobilisation.
Do I need a referral?
No. You can book directly without seeing a GP first, which is what most people do. If you are claiming on private medical insurance, check your policy first because some insurers still require a GP referral before they will authorise treatment.
